Service: Bariatric Registry

Description (Service) Designed as a centralized referral system, as well as a standardized database collecting information on patients undergoing bariatric treatment within Ontario.  
The centralized referral process will help coordinate care efforts, resulting in less duplication and shorter wait lists. 
The Bariatric Registry will also help maximize the effectiveness of health care spending on obesity treatment and identify best practices providing a starting point for new policies and strategies surrounding standards of care. The creation of the bariatric registry is helping to achieve the goal of more efficient patient treatment.

Public Comments The Bariatric Registry is intended for referral of ELECTIVE patients ONLY.

Areas Served Ontario
Eligibility Ages: 18 year(s) and up 
• BMI of 40+ or a BMI of 35 plus 2 co-morbidities 
• Patient has a valid Ontario Health Card 
• 18 yrs of age or older 
• Elective patients only 
Physician or health care provider must make the referral for their patient.
Application Physician provides referral by either printing the referral form available online and faxing it or by making the referral over the phone * the respective centre will contact patient to schedule first appointment and orientation session 
Urgent post-operative care situations - contact your local hospital/Bariatric surgeon for immediate attention of the patient.
