DescriptionRegisters births, marriages, deaths and name changes; maintains adoption records. Provides birth, marriage and death certificates. Apply online at www.ServiceOntario.ca.  
 
Maintains a list of religious officials who are authorized to perform marriages in Ontario at www.ServiceOntario.ca 
 
You may obtain a birth certificate application form from Municipal Offices, Provincial Government Information Centre (GIC), or you can download form or submit online form electronically from our website at www.ServiceOntario.ca.  
 
The Office of the Registrar General offers four levels of service for certificate applications: online, regular, expedited or emergency.  
 
ONLINE BIRTH CERTIFICATE applications for individuals may be completed online with payment by credit card. Processing time is guaranteed to be within 15 business days of receipt of the application, plus delivery by regular mail or your money will be refunded. The premium online service is 5 business days including delivery by courier for an additional $30 per certificate or your money will be refunded. Applicants 9 years of age or older must provide guarantor information. A signature from the guarantor is not required, however the applicant must obtain the guarantor's permission to provide his or her contact information. Births, marriages and deaths must be registered to qualify for this service.  
 
REGULAR SERVICE applications for a birth, death or marriage certificate take 6-8 weeks to process from receipt of the application.  
 
EXPEDITED SERVICE applications for a birth, death or marriage certificate are completed within 10 business days of receipt of the application plus delivery time. Birth certificate applications for Expedited Service along with proof of urgency may be faxed to (807) 343-7411, provided the applicant is paying by credit card.  
 
EMERGENCY SERVICE is only available for births registered since 1930, marriages registered since 1965 and deaths registered since 1980. Birth certificates are couriered within 2 full business days from receipt of the application. This service is only available in person at the Downtown Toronto ServiceOntario Centre at 777 Bay Street, College Park. Expedited service and Emergency Service is available only to those persons who provide proof of urgency and submit a completed birth certificate application form. Please note, a processing delay may be experienced if any part of your application is incomplete.  
 
Individuals must complete the Statement of Guarantor section of all birth certificate applications. A guarantor must be a Canadian citizen who has known you personally for at least two years, is confident that the statements made by you are true and is available in the event that the Office of the Registrar General requires additional information.

Ontario's adoption records are now open since June 1, 2009. This means that adopted adults and birth parents can apply for post-adoption birth information from birth records and adoption orders. If you want your identifying information kept private, and if the adoption order was made before September 1, 2008, adopted adults and birth parents can file a disclosure veto.
